Alabama is preparing to launch its medical cannabis program, offering patients with serious or chronic health conditions access to regulated, physician-recommended cannabis therapies. While the program is still in development, the law lays out who can qualify, what forms of cannabis will be allowed, and how much patients may possess.

Qualifying Conditions: Alabama Medical Cannabis Card – Online Evaluation | Medical Card Exam™

To be eligible, you must have one of the following conditions and documentation from your physician showing that traditional medical treatments have failed:

• • Autism Spectrum Disorder (ASD)

  • Cancer-related cachexia, nausea, vomiting, weight loss, or chronic pain

  • Crohn’s Disease

  • Depression

  • Epilepsy or seizure disorders

  • HIV/AIDS-related nausea or weight loss

  • Panic disorder

  • Parkinson’s disease

  • Persistent nausea not responding to standard treatment (excluding pregnancy-related nausea, cannabis-induced vomiting, or cannabinoid hyperemesis)

  • Post-Traumatic Stress Disorder (PTSD)

  • Sickle Cell Anemia

  • Spasticity from ALS, MS, or spinal cord injury

  • Terminal illness

  • Tourette’s Syndrome

  • Chronic or intractable pain where opioids are ineffective or unsafe

Patient Possession Limits

• • Patients can hold up to 70 daily doses at a time.

  • THC caps: No more than 50 mg per dose during the first 90 days.

  • Only non-smokable forms are allowed:

  • Tablets, capsules, tinctures, gel cubes (oral use)

  • Gels, oils, creams (topical)

  • Suppositories, transdermal patches

  • Nebulizers or inhaler-compatible liquids/oils

  • Patients under 18 may only access low-THC cannabis products.

Home Cultivation

❌ Not permitted in Alabama.

State-Licensed Dispensaries

✅ Yes — licenses will be issued, but no dispensaries are currently operational.

Delivery

❌ Not allowed.

Caregivers

✅ Allowed — caregivers can assist registered patients.

Estimated Number of Patients

The registry is not yet open, so patient numbers are currently zero.

Employment Protections

❌ No protections for qualifying patients at this time.

Reciprocity

❌ Alabama does not currently recognize out-of-state medical cannabis cards.
